To request absentee ballot either send email, mail to [email protected].  or go to http://myvote.wi.gov. 

You must state your name, your address, the election or elections you are requesting the ballot(s) for and INCLUDE A PICTURE OF YOUR PHOTO ID.  Your ballot will be sent out the next day.  BEFORE YOU RETURN YOU BALLOT MAKE SURE YOUR CERTIFICATE ENVELOPE IS FILLED OUT COMPLETELY. SIGN YOUR ENVELOPE IN FRONT OF A WITNESS AND HAVE THEM SIGN AND ADDRESS THE CERTIFICATE AS WELL. YOUR BALLOT MAY BE REJECTED IF ALL ARE NOT COMPLETED.
